Planning for life after school is not an isolated event that, once it is done, it is over. It is an ongoing process that changes and evolves over time.
The Future Pathways Guide 2007 is a transition guide to assist ACT students with a disability make informed decisions and plans for life after school.

The guide is designed to be a working document for use by Year 9-12 students and their families. It is also useful for disability services providers, student welfare teams, teachers, counsellors, careers advisors, apprenticeships coordinators and service providers.
The guide is a project of the ACT Interagency Transition Committee and was launched at the 2007 ACT Post School Options Expo on 22 May 2007. This ACT Interagency Transition Committee was established in February 2006 to increase cross-agency collaboration within the disability sector so as to improve post school outcomes for students with a disability transitioning from school and college to adult life.
