What is an Individual Learning Plan? |

Individual Learning Plans are working documents that:
- Outline short and long term learning goals as well as the actions, strategies and adaptations that may be used to support students to achieve their goals
- Assign responsibility and accountability for implementing and evaluating the learning plans
- Provide an ongoing record of outcomes to improve continuity of learning over the course of a student's time at school
- Promote a collaborative planning process involving a team of people who know and can actively participate to identify and plan for the student's individual needs and priorities for learning. Core participants of the team include:
- Teachers
- Specialist School staff
- The student
- Parents and carers
